Medication and Procedures Involved
(Please do not try self medication in any case! Consult your primary GI)
Procedures to detect severity:
Normally your GI will suggest you to do Colonoscopy first time to get to know the ulcers and area where the infection is spread. It will help GI to form opinion if it is IBD, UC or Crohn's. GI will additionally may take small tissue sample for biospy.
Subsequent examination might not require full colonoscopy but can be restricted to Sigmoidoscopy.
What patient need to follow before procedure?
Colonoscopy : laxative powder is given in 1.5 L water or soda(I don't recommend soda for UC) on the day of scopy morning which will clear your bowl (7-8 times watery motions). Please remain hydrated with clear water, lemon water, sugar+salt water. You will feel pale due to frequent motions.
Sigmoidoscopy : laxative tablets like four dulcolax will be given after dinner of night before scopy to clear the bowl.
Medication:
Once the biospy and scopy results are out your GI will start the treatment with below conventional path of medication. (note the path is lower to higher)
- mesacol Tab 800 X 4 a day (generic drug Mesalamine) if it is not controlled then 6-8 Tabs a day alternate is Rowasa 1mg sachet from Abott (~ INR 31 1gm sachet).
- Happi 20 or Pan 40 tablet (~ INR 40 10pc)
- some pre and pro biotic like Bifilic HP (~ INR 160 10 pc)
- Mesacol enema or entafoam enema (~INR 180 1 pc)
- Azoron 100 mg generic drug Azathioprine (~INR 100 for 10 Tabs)
- Wysolne (pred) steroids starting with 10mg up to 50mg per day to manage flair (~INR 14 10mg 10pc) (*only during flair management but body get used to it and becomes dependable, so taper down as soon as you can)
- Remicade 100 mg with IV (need at least day care admission to hospital) It costs 40,000 INR for one shot. (*only during flair management once you get one shot then have to continue on Azoron forever)
